Practice Policies & Authorization to Bill

Appointments & Cancellations

  • Initial visits are scheduled for 60–90 minutes
  • Follow-up visits typically last 15–30 minutes

To hold your appointment, payment must be received within 24 hours of booking. If payment is not received, your time slot may be released to another patient.

We require at least 48 hours’ notice for cancellations or rescheduling. Cancellations made less than 48 hours in advance, or missed appointments, will be charged in full.

Your card may be automatically charged 24 hours prior to your appointment if payment has not yet been submitted.

If you arrive late, your session may be shortened to honor other scheduled appointments.

We aim to respond within 24 hours, though we are not always immediately available.
Please note: Face-to-face video visits are preferred for quality of care, but phone sessions may be available in certain situations (travel, illness, support needs).

Electronic Communication & Telemedicine Consent

We offer care primarily through telemedicine, including video, email, and messaging platforms. While we take every reasonable step to safeguard your information, no form of electronic communication can be guaranteed 100% secure.

By using our services, you acknowledge and understand the following:

  1. Telemedicine is voluntary. You may withdraw consent at any time. However, because Pink Pearl Primary Care is primarily a telehealth-based practice, this may result in discontinuation of care.
  2. Your health information is protected with the same care as a brick-and-mortar clinic, but we cannot eliminate all risks of digital communication.
  3. We do not use your information for research unless you provide specific, written consent.
  4. Telemedicine has benefits and limitations. While it improves access and reduces travel, remote care limits full physical exam capabilities, which may affect diagnosis and treatment planning.


Minors

Parental consent is required for all visits involving minors. A parent or guardian must be present during a portion of each visit to confirm consent.

If you are a minor, please note:
Your parent or guardian may be legally entitled to access some aspects of your care. We will discuss together what information is appropriate to share and what may remain confidential.


Termination of Care

Pink Pearl Primary Care reserves the right to terminate care at any time, with cause and proper notice. Reasons may include:

  • Repeated missed appointments
  • Failure to complete recommended labs
  • Ongoing non-compliance with treatment
  • Withdrawal of telehealth consent without alternate care arrangements

We will always make reasonable efforts to discuss the reason for termination and provide referrals or alternative provider recommendations when applicable.

PRACTICE POLICIES AND PROCEDURES

Purpose:
To ensure adherence to all applicable laws, rules, and regulations while ensuring patient access to Direct Primary Care and associated health services through the use of live two-way audio-visual communication. By implementing these procedures, Pink Pearl Primary Care provides distance-based medical services in a safe, secure, and effective manner.


I. DEFINITIONS

  • Direct Primary Care (DPC): A cash-pay (non-insurance) healthcare model offering prevention, wellness, and treatment for most routine medical needs.
  • Associated Health Services: Services provided by Nu Forme Wellness, such as weight management, functional medicine, and therapeutic support.
  • Telemedicine: The delivery of healthcare using technology, including live video, phone, email, or other digital tools. The same clinical standards apply as with in-person care.
  • Medical Providers: Licensed MD, DO, NP, or PA.

II. APPLICATIONS / SCOPE

Services Provided via Telemedicine

Conditions commonly managed include but are not limited to:
Acid Reflux, Acne, ADHD, Allergies, Anemia, Anxiety, Arthritis, Asthma, Birth Control, Cold & Flu, Constipation, COPD, Depression, Diabetes, Ear/Eye Infections, Fibromyalgia, Gut Imbalance, Headache, Heart Palpitations, Hypertension, High Cholesterol, Muscle Tension, Neuropathy, PCOS, Preventive Care, Skin Rashes, STIs, Sinusitis, Thyroid Disorders, UTIs, Weight Management, Vitamin Deficiencies, etc.

Service Locations

Telemedicine services are performed remotely by both provider and patient. In-person evaluation may be recommended when medically necessary.

Clinical Oversight

Chart audits and peer consultations ensure quality care.

Contraindications

If sound/video is inadequate or a patient’s condition requires a physical exam, in-person referral may be advised.


III. PROVIDERS

  • Licensed in the state of service
  • Minimum 1 year of clinical experience
  • Board-certified; DEA license optional
  • May decline to prescribe controlled substances as clinically appropriate
  • Must complete training and CME where applicable

IV. PRIVACY, CONFIDENTIALITY & SECURITY

  • Both provider and patient must be in private settings
  • “Do Not Enter” signage is recommended during sessions
  • Non-clinical observers are discouraged
  • No audio/video recording without written consent

V. INFORMED CONSENT

  • Patients must provide written informed consent before services
  • Consent includes risks, limitations, and alternatives
  • Refusal of telemedicine may result in referral to another provider
  • Consent is documented in the EMR

VI. REQUIRED DOCUMENTATION

  • All visits documented in the cloud-based EMR
  • Progress notes completed within 72 hours
  • Labs and prescriptions must be logged
  • Equipment issues must be noted in chart if care is disrupted

VII. MEDICAL RECORDS

  • Stored securely in the EMR
  • Accessible for routine and emergency care

VIII. TRAINING, LICENSURE & LIABILITY

  • All providers and staff must demonstrate EMR competency
  • Background checks and license verification required annually
  • Nu Forme Wellness provides malpractice coverage; additional personal coverage is encouraged

IX. EQUIPMENT FOR VIDEOCONFERENCING

  • Providers must use a secure, video-enabled device compatible with the EMR system

X. OPERATING PROCEDURES & SCHEDULING

  • Patients book appointments via the online portal
  • Phone support available for general questions and scheduling help
  • Patients select providers based on their location and availability

XI. PROVIDER RESPONSIBILITIES

  • Respond to clinical messages within 24 hours
  • Place and manage lab orders, review prior to follow-ups
  • ePrescribe as clinically appropriate
  • Coordinate in-person visits when controlled substances are prescribed
  • Stay current on DPC standards
  • Educate patients on maintaining local PCP care
  • Obtain physical exam documentation from outside provider if needed
